The information you provide on your egg donor application is for use by recipient parents in order to select their ideal egg donor. The information you provide to our surrogacy team will remain private and confidential; all personal information regarding your name, address, personal medical and health records, etc. will not be provided to these individuals.

Thank you for taking the first step to helping an infertile couple realize their dream of having a family. The following online form is the first step in that process, here are a few important points to cover:
- The information you provide will be presented to prospective parents. Please ensure that you answer all of the questions completely while being as thorough as possible.
- PHOTOS: Regarding photographs.. the more the better. Color is best; be sure to include both a face close up and at least one full body photo. We are looking for baby, childhood, teen and adult photos. Original work is great, they are much clearer when scanned. You can email your pictures to Glory@conceptualoptions.com or you can mail your photos to our San Diego surrogacy agency, please include a self-addressed stamped envelope if you would like your photos returned.
- PAP SMEAR: You must provide us with a “normal/negative” Pap Smear result that is less than 1 year old! If you are due for one, please make the appointment as soon as possible.
- Normal turn-around from the time we receive your inquiry until you are on our Database and being viewed by Intended Parents is 5 business days.

“Egg donation involves a screening process. Not all potential egg donors are selected. Not all selected egg donors receive the monetary amounts or compensation advertised. As with any medical procedure, there may be risks associated with human egg donation. Before an egg donor agrees to begin the egg donation process, and signs a legally binding contract, she is required to receive specific information on the known risks of egg donation. Consultation with your doctor prior to entering into a donor contract is advised.”
